Job Title: Therapeutic Foster Care Coordinator

Report to: Therapeutic Foster Care Supervisor
The role of the Therapeutic Foster Care/Respite Coordinator is to provide direct services and support to children placed in foster homes and foster parents within the agency. TFC Coordinators help coordinate services among involved professionals and work to expand and enhance the program as a whole. Services include supporting children and adolescents within short-term and long-term placements and ensuring that they are receiving individualized services that meet their needs. This position is to be sensitive, respectful and responsive to the cultural background and values of each family served. This position reports to the TFC/Respite Program Supervisor. This position requires a Bachelor’s Degree in the social service field. Experience is Preferred.
Specific responsibility includes but are not limited to:
- Maintain compliance with applicable rules and regulation.
- Maintains placement file (by the end of the month) and documentation (within 48 hrs. of visits) Correctly and in a timely manner.
- Recruit new providers.
- Process intake information and place children referred with appropriately matched providers.
- Coordinate with staff and providers to gather any missing data on children placed within foster homes.
- Coordinate and collaborate with representatives from outside agencies.
- Provide monthly summaries to DFCS, DJJ (by the 10th of the month) and all applicable parties (MAAC by the 3rd of the month).
- Attend monthly foster parent trainings/meetings.
- Facilitate monthly foster parent trainings/meeting as needed.
- Obtain and maintain CPR/First Aid certification.
- Conduct and complete documentation for “Monthly Every Child Every Month” visits before the 10th of each month. (48 hours after visits occur)
- Ensure the GA Score system is updated by the 5th of each month with required information for children/adolescents.
- Ensure that new clients are placed in GA Score within 5 days of placement
- Attend intake for new clients assigned to caseload.
- Attend court, panel reviews, school meetings, etc. for children placed within the foster home.
- Availability for crisis situations by phone and if necessary conduct a visit to stabilize the situation. This includes but is not limited to, after hour calls, emergency hospital stays and crisis that may occur on the weekend.
- Complete incident reports for ORCC within 24 hours
- Complete interviews and collect written statements from involved parties within 48 hours of reported incidents.
- Conduct ISP planning meeting every 180 days
- Work in partnership with other employees.
- Other duties as assigned.
